Owner & Senior Care Manager
LifePath Senior Care Management was founded by Shannon Busch with a simple purpose: to provide older adults and their families with a trusted, knowledgeable resource as they navigate the changes, decisions, and challenges that can come with aging.
With extensive experience in social services, long-term care, healthcare, and senior care management, Shannon has spent her career supporting older adults and families through changing care needs, care transitions, discharge planning, dementia-related concerns, family decision-making, and planning for safety and independence at home.
As a Certified Social Worker, Certified Dementia Practitioner, and Senior Home Safety Specialist, Shannon brings together professional experience and a compassionate, personalized approach. She takes the time to understand each person's needs, preferences, routines, and goals—because aging support should never be one-size-fits-all.
Through LifePath, Shannon provides Senior Care Management, Wellness & Companion Visits, and Home Safety Assessments, along with advocacy, care coordination, family guidance, and aging-in-place support. Each service is designed around a shared goal: helping older adults remain safe, connected, independent, and well-supported, while giving families greater confidence and peace of mind.

Senior Wellness Visits & Companion Services provide regular companionship, wellness and safety check-ins, social engagement, transportation to appointments, grocery shopping and errands, medication reminders (non-medical), and family updates for added peace of mind.
Every family is unique, so services are personalized to meet your loved one's individual needs and goals.
We proudly serve Ocean County, NJ and surrounding areas. Some services may also be available virtually depending on the situation.
Senior Care Management is a personalized service that helps older adults and their families navigate the aging process. We assess your loved one's needs, provide guidance, coordinate resources, advocate when needed, and develop a plan to help them remain safe, independent, and well-supported.
Wellness Visits are scheduled, one-on-one visits that provide companionship, social interaction, wellness and safety check-ins, and reassurance for both older adults and their families. Each visit is personalized to meet the individual's needs and can include errands, transportation to appointments, medication reminders (non-medical), and updates to family members.
No. LifePath Senior Care Management does not provide personal care such as bathing, dressing, medication administration, or skilled nursing. We specialize in Senior Care Management, Wellness Visits, companion support, care coordination, advocacy, and guidance. If personal care is needed, we can help connect you with trusted local resources.
Our services are ideal for older adults who wish to remain independent, families caring for an aging loved one, adult children who live out of town, caregivers who need guidance, and anyone planning ahead for future care needs.
Absolutely. We encourage family participation and work closely with loved ones to develop personalized care plans, provide updates, and ensure everyone feels informed and supported. Family consultations can be conducted in person, by phone, or virtually.
Wellness Visits can be scheduled weekly, biweekly, monthly, or as needed based on your loved one's preferences and level of support required.
LifePath Senior Care Management is a private-pay service and does not bill Medicare or private insurance. Payment is due at the time services are provided unless other arrangements have been made.
